What does pedal edema indicate?

Pedal edema (foot and ankle swelling) is one of the cardinal signs of congestive heart failure (HF) but can also be due to other systemic or local conditions, including chronic kidney disease, liver disease, thyroid disorders, venous insufficiency, and venous thrombosis1.

Is pedal edema serious?

Edema in foot and ankle Swelling of the foot, ankle and leg can be severe enough to leave an indentation (pit) when you press on the area. This swelling (edema) is the result of excess fluid in your tissues — often caused by congestive heart failure or blockage in a leg vein.

What side of heart failure causes pedal edema?

When the left side of the heart is failing, it can’t handle the blood it gets from the lungs. Pressure then builds up in the veins of the lungs, causing fluid to leak into the lung tissues. This may cause CHF and pulmonary edema.

What causes foot edema?

Swelling in the ankles, feet and legs is often caused by a build-up of fluid in these areas, called oedema. Oedema is usually caused by: standing or sitting in the same position for too long. eating too much salty food.

How to score edema?

Edema Grading. Pitting edema is graded on a scale of one to four. The scaling depends on both the “pit” leaves and depth and how long the pit will remain.

How to rate Edema?

Water displacement and ankle measurement more reliable methods

  • Clinical assessment highly variable due to its subjective nature
  • Bed-bound supine patients the interstitial fluid accumulates at the sacrum
  • Assess how far up the body the edema goes (1+pitting edema on the chest wall may be more significant than 3+pretibial pitting edema)
  • How to chart pitting edema?

Assess for pitting edema. Press edematous area firmly with thumb for 5 to 10 seconds. Remove thumb; note extent of indentation and how long it lasts. 7. Measure edema on a scale of 1+ to 4+ edema. You may use a tape measure. a. 1+ edema = 2 mm induration.
